About Senior Care Planners (SCP)

Who are We?

The SCP. team comprises highly skilled Registered Nurses with decades of professional expertise. Our company provides extensive clinical insight into the sometimes baffling and complex aspects of the Continuity of Care process. We help to customize and support optimal choices that meet each client's unique needs. Our services are tailored to the specific circumstances of each individual client.

How does it work?

  • We meet with the client and others designated to be involved with their care and decision-making.
  • We provide a complete assessment of the physical, mental, emotional, and financial status of each client.
  • We recommend and implement a plan of care that is agreeable and manageable for all involved.
  • We regularly adjust that plan in response to the changing care needs of the client.

Services Offered

  • Complete initial assessment of the client's needs in collaboration with their caregivers and/or providers.
  • Development of a plan of care and coordination of services to meet the needs of the client.
  • Accompanying the client to physician visits and assisting in describing the current level of functioning and any issues that are of concern.
  • Assistance in preparing medications by pre-pouring pills or collaborating with others who may be administering medications.
  • Arrangement for care in the home based on the intensity of service and client needs. Care may include professional nursing, therapy, or counseling services in addition to supportive care services.
  • Arrangement for a client to be admitted to a nursing home or assisted living facility as necessary.
  • Assistance with arrangements for a client to return home based on current needs after a nursing home or hospital visit. 
  • Collaboration with attorneys, financial advisors, and others to assist with and coordinate the care and support needed as changes occur in the client’s life or lifestyle.
