Services Offered
Services offered may include:
- Complete initial assessment of the client's needs in collaboration with their caregivers and/or providers.
- Development of a plan of care and coordination of services to meet the needs of the client.
- Accompanying the client to physician visits and assisting in describing the current level of functioning and any issues that are of concern.
- Assistance in preparing medications by pre-pouring pills or collaborating with others who may be administering medications.
- Arrangement for care in the home based on the intensity of service and client needs. Care may include professional nursing, therapy, or counseling services in addition to supportive care services.
- Arrangement for a client to be admitted to a nursing home or assisted living facility as necessary.
- Assistance with arrangements for a client to return home based on current needs after a nursing home or hospital visit.
- Collaboration with attorneys, financial advisors, and others to assist with and coordinate the care and support needed as changes occur in the client’s life or lifestyle.
